It's not difficult at all. You do a constant addition/subtraction of +/- 1 as the cards come out, if you practise at home it becomes second nature after a bit. Keeping a true count on the deck is pretty easy, its when you get into division based on decks that it gets difficult.
True Count is a pretty good indicator where the deck is at, combined with how many cards are left (rough estimate).
