I'm sure it was. But the honeycomb effect is due to the concrete being improperly placed, as with the rebar not being tied correctly. Both causing decreased compression strength.

Easily. Those saying the engineers will be fine must be forgetting that their insurance is about to triple. They might be okay for a little while but this is gonna follow them and could eventually destroy the firm.
