having hell hitting my mash temps lately. hit 149 instead of 152.

questions for yall
in one recipe I see the following mash schedule:
Mash Schedule
-----------------
Sacch 45 min @ 149F
Sacch II 15 min @ 161F
does that mean raising the temp of the mash or adding more water to raise the temps?
whats the goal behind this, to increase FG?
If you do this, does the water for the second step come from the original mash volume or the sparge volume?
Does it mess up your efficiency?
