Just checked my notes. Brewed 1/13/13, so the fermentation temp would have been in the low to mid 60's at the most.
I did a 5 gallon boil, but I know I didn't take into account evaporation, and when I bottled I only had about 4 gallons.
One thing I noticed in my notes is that my target fg, according to brewers friend, was 1.021, but I measured 1.011. Big difference and probably accounts for some of the booziness
Can't wait to brew it again, with a year of experience and a much better brewing calculator like beersmith
eta: when I put this into Beersmith, it gave an estimated FG of 1.013, which is much closer to what is actually was
