I worked with NextGen in a project management capacity. It was expensive but affordable but this was in mult-specialty environments with multiple locations. The hardest part was interfaces from the hospital systems for ADT and lab. You probably didn't have to do it with a solo practice but if you made mistakes on the front end design with the hierarchy, then you were screwed. I didn't care for that.
