Not into major cities, but would like to see Rome if it fits.

More into mountains and hiking/eating and drinking.

Not dead set on Italy, but it’s a place we both would like to go to.

Two words. The Dolomites. Stunning and food is a blend of Germanic and Italian. Then you could spend a couple of days in Venice as a gondola ride is just about the most romantic thing you can do in that part of the country. Then tran to Lake Como or Lake Garda. Northern Italy at it's finest. Personally I'd also get over to the Piedmonte region but I am a sucker for the food (white truffles) and wine (Barolo and Barbaresco) there.

The Dolomites are truly one of the most amazing places in the world.

You could do Milan, Lake Como, and the Italian Alps in Bormio as a good trip too.
