It was formally dogmatized as canon at the Council of Trent. It had always been in the Bible since the Latin Vulgate though, which was the first Bible, created by the Catholic Church in the 4th century.

It was added in the 16th century to justify indulgences.

It has not always been in the Bible, and nobody other than the Catholic Church considers it inspired, not the Jews or any other Protestant denomination. Jesus never drew from it, not once.
