The size of the implant is determined by the the length of the two parallel cylinders that make up the erectile tissue of the penis.
During the surgery those two cylinders, that fill with blood and expand during a normal erection, are quite literally reamed out with dilators until they are basically hollow. The prosthetic is two inflatable tubes that replace that newly hollowed area in the penis.
A pump is then placed in the scrotum, a reservoir of fluid in the lower abdomen. When it’s time for sex, you pump it up. Fluid goes from the reservoir and inflates the two new cylinders.
It’s a brutal surgery.
Extra length is impossible. You are confined to the original length of the erectile tissue.
