Reminds me of the story I read by a trans woman who had bottom surgery and said it’s “vag” constantly smelled like feces and was causing health problems.

That may be some of it but I think the vast majority of it is the "no harm" theory of care for mental/emotional/addiction issues. To these believers, anything which makes the person with issues more uncomfortable (like arresting a homeless addict for shitting on the sidewalk, for example) is to do harm to them.
The over-empathetic nature of this mindset creates more propagation of problems through avoidance than actually curing the issue.

He is 18. I have no issue with him choosing to have a risky operation. I have a huge issue allowing children that option.
