Any insight and/or suggestions for a fix?

get water in the outlet at any point? Have you tried a different set of headphones?

New headphones first. Try to see if the same issue occurs. If so, then canned air into the headphone jack. There may be lint or dirt in the female end in the iPhone.
If both those crap out, check the Apple Forums. If still no help, then go ahead and schedule a service appointment at the Apple Store.

Haven't had any water damage. And I've tried two sets of Apple earbuds with the same result. The problem doesn't happen when I connect my phone to the car radio or to an external speaker. Apple Forums haven't been any help.
Phone is about 6 months old now. Is it still covered by the standard warranty?
