Have you ever really gotten revenge on someone?

I was thinking about the phrase "Revenge is a dish best served cold." I can't say I've ever had something done to me that I needed to exact revenge for.

No. Anything that would warrant revenge from me is also likely a felony, so the police would/should take care of it for me.

My old room mate would sometimes forget to lock the door. So I got his girlfriend pregnant and made him think it was his. I'll tell him when he's 18.

While living in dorm room in college, my freshman year roommate was an inconsiderate punk. Therefore, I pissed a little in his bottle of 'face refreshing liquid splash'. He used it twice a day and I enjoyed it every time he splashed my piss on his face.
