Fantasy Football Hypothetical - Verdict?

I was thinking about this this morning and I'm sure it's happened to SOMEBODY.
Scenario: You propose a trade for a player. While waiting to hear back the player gets suspended or hurt for a long period of time or even the year.
You go to cancel the trade offer.....and the guy had accepted after hearing the news.
What is fantasy football protocol here?

its still a trade.. ive seen it happen! Almost happend to me yesterday in the other 20 man, i had proposed lamar miller to trade earlier in the week.. i got on fast yest to cancel request

Shitty of the other owner to accept, but within his rights.
This is why you never blind propose a trade. Always work out trades through email/text/chat and only propose when a deal if verbally agreed upon. And dont leave proposals just hanging out there.

Let's say.
1. Trade proposed.
2. Car wreck and guy loses his leg.
3. Other guy hears news first and accepts really quick.
Should commish step in?

However, it's hard to prove that the other dude saw the news first and then went and quickly accepted.
Material change in circumstances, I would want the trade cancelled, whether I was involved or not.
