Jefferson parish has a parish president, while Orleans has a mayor....why the different use of terms? How is the term determined?

Orleans Parish and New Orleans are coterminous - i.e. one and the same, geographically. There really isn't a "Parish" government in Orleans - its just a city government. It wouldn't make sense to also have a Parish government because they'd be governing the exact same thing.

Becase Orleans Parish is no longer distinguishable from the city of New Orleans, New Orleans could be considered, in form, an independent city.

Metaire is not a city.

(nor a town, nor a village...)

Metairie has no existence whatsoever outside of being a "Census Designated Place" - that's right, were it not for the U.S. Census - Metairie would not even exist.

Metaire WOULD be one of the state's largest cities were it to incorporate - which it won't - this is why the Jefferson Parish government is pretty important compared to a lot of other parish governments, in terms of the numbers of constituents in unincorporated areas I'm guessing it has the most of any Parish.
