Address it to the tenant with the current date

You are hereby notified to vacate the premises described in the address above within (5) days of the date of the delivery of this notice to you, as your lease was terminated when you failed to pay the current rent when it was due. Should you fail to vacate within this period, court proceedings will be taken immediately to evict you from the premises.

Sign and Print your name as landlord
Have two witnesses sign it.
Post on door of the evictees address.
Take picture up close and farther out to capture property.

Worked for me.

If you have the correct language in the lease, you will not have to post the 5 day eviction notice and may go straight to the justice of the peace. Check your lease language.
