Worst choices for replacement actors in a sequel (and best if you have some)

For worst, I'd say Jon Voight for Tommy Lee Jones for the role of Woodrow F. Call. Voight is not a bad actor but he was out of place trying to fill Jones' place as the bad assed Call.
Best is harder for me to come up with. But if, as I've read there was someone (Brian Cox I think) who played Hannibal Lector before Anthony Hopkins, then I definitely think the choice of AH for Silence of the Lambs has to rank pretty high up there.

I carry a severe grudge against Game of Thrones after they recast the Mountain with a guy that looked like Vlade Divac in season two.

Absolutely.
Also, even though she was hotter, I didn't like April O'Neal in TMNT2 as much as the one in the first one. For some reason, Judith Hoag just fit to me.
