| One thing about the Medicaid expansion that is left out (Posted on 11/19/12 at 8:46 am)
leave out the hospitals. WHY would a doctor be for it? Its going to screw them. Same thing with therapy providers. The cost of treating a patient is usually more than the reimbursement or the same as thee reimbursement from Medicaid. Its a money losing proposition for them. The expansion will take away patients who are either paying for services or the insurance company pays. Don't they realize it means doctors will start refusing to treat medicaid patients?

START refusing? And yes, you're right on the money. You are expanding medicaid's rolls, but not increasing reimbursement, in an already undersupplied field of medicine. Which is why the whole "ACA creates access to care for millions of new patients" is actually complete and total bull shite. This also intertwines with all of the recent mid levl provider threads. County hospitals, and other similar facilities, will begin using mid levels IN LIEU (sp?) of physicians instead of with physicians. Mistakes will happen. Lawsuits will happen.

Step 2: pass law forcing doctors to accept public health care recipients. Profit???

Nah, it will be much more incentive laden. Not a threat. Something like...physicians practicing in state facilites, and employed by the state, wil be offered amnesty from medmal litigation. That can cover GP's and specialists, and is an expense > $100,000/yr in most states. You'll have plenty of takers. Or connect the Medicaid and Medicare programs to where you can only take both or none. But not just Medicare, which is slightly profitable right now.

I was talking to a therapy provider last night. The expansion will rob them of their private patients. Its a low margin industry anyway. No one gets rich off of this service although they can live ok. They make a profit of $1-2 per visit from a medicaid patient and sometimes lose money on that visit.
