What's a fair price to expect for getting a cement slab framed and poured?

It is just a back yard slab to extend back patio slab.
Won't have any vehicles driving on it.
The area is exactly 500 square foot.
Any idea what this would cost?

$4-6 per sqft. In Alexandria I recently paid $4

You have 6 cubic yards of concrete at 4 inches thick. Figure $105/ cubic yard here, but you're most likely going to have to pay for a full truck load, or 8 cubic yards. You have $840 plus tax here, using the 105 unit cost.
You will be safe to pay someone $2/ SF to form, place, and finish on that small of a slab. This gets you another $1,000.
Figure $200 or so for form lumber.
Are you putting mesh in the slab? Another $200 will cover you here.
Will you haul off the removed dirt/ soils from the excavations?
$2,500 to $3,000 will cover you, depending on your location.

Take the price of the concrete and double it.
