Someone please explain this product to me.

So you rest a barrel pointing down straight at your foot... am I missing something? Looking at it from a safety standpoint, it doesn't seem too smart.

People with O/U use those. Only people I've seen with them.
And yes, everyone I've seen with them had the O/U broke open

It is for hardcore trap and skeet shooters, they rest the barrel on their foot like that between rounds. They are shooting o/u typically so they are broken open and cannot fire so no risk of a discharge in the foot.

Looks to be like a leather thingamajiggy that some folks have on their key chain.
Laces on boots intertwine with the loop and it fits on top of foot part of boot. Your barrel rests on that, I think?
Although, if you aren't shooting in lace ups, or tenny shoes, a piece of plywood duck taped to your shoe will have to suffice...

I just put mine on my toe but I guess I never stood like that long enough to be bothered by it.
Fun fact: one of my friends shot off his pinky toe that way.
