I received a bill from "Our Lady of the Lake" for an "out patient procedure" that happened on a date I know I was at home. Also, I have NEVER been a patient at the lake. The bill asked for credit card info only, no check, no money order, just credit cards. I checked the call back number which was provided so that you can call and give your CC info on the bill and the info online said it was a scammer. It certainly was not connected to the Lake. The woman I talked to at the Lake said to call the police and a quick google search said to contact the Louisiana FBI office.
What the hell should I do with this thing?
Do I really call the police or FBI? Does any OTwr have any experience with stuff like this?

The only reason I am concerned is that they had the last 4 digits of my social correct, but did have my middle initial incorrect.

Be sure you make a report with the local law enforcement wherever you live. They probably ain't gonna do anything but maybe refer you to the FBI or some such federal agency but at least you would be on file with a bonafide law enforcement agency so you can prove this to everybody who is about to come after you for claiming you owe them some monty because someone has stolen your identity and scammed them and you.

One other good thing to do, just to mess with them. If you have a prepaid Visa card that's mostly depleted, call and give them that number.

Worst case, you waste a few minutes of their time. Best case, they confirm the card by running a $1 charge, then sell it to someone in the Russian mob, who comes back and kills them when they realize the card is bogus.

Sounds like it's up to you to bring these frickers to justice on your own, Liam Neeson style.

I was contacted by my cousin, who happens to be a nigerian prince a while back...all he needed was a few bucks for a plane ticket and we would both be wildly rich...

the local police referred me to the FBI. I reported to them and never heard anything more about it...

Troll the scammer and report back to the OT, hopefully on a day when I'm bored at work.
