His troops were already threatening mutiny in India. Alexander had to turn back. The problem was Alexander had no desire to actually maintain his empire, only expand. Had he lived, his army would have demanded to return home for a couple years. He would have had to basically build a new army. He proved that he had the ability to lead his troops, but would he be able to keep the peace? What happens when his armies aren't marching across Afghanistan and the locals decide to stop paying taxes? What does he do if while putting down that revolt, Egypt flares up? Could he hold it together, rebuild his army, build the infrastructure of the empire, and then press forward and finish off the Indians and Chinese? I just don't see it.

Alexander's lack of an heir showed his inability and complete lack of desire to do what was necessary to maintain an empire. That's why he is known as a great conqueror and not a great ruler. The empire was always going to die with him.

That's why it's called a hypothetical. In this scenario it's assumed his army keeps going and keeps winning. As for getting to South America, he'd probably hitch a ride with the alien astronauts. OP knows about those.

First, his army goes home to Greece. Alexander chills in Babylon and one of his generals or family members revolts in Greece. Alexander is forced to raise an army of Persians and eastern conscripts. He puts down the revolt, but not without large losses. These losses combined with his retreat from India destroy the air of invincibility. When he goes to put down the revolt in the West, Afghanistan revolts. Alexander puts it down. For 10-15 years, Alexander spends the Empire's money on building roads and putting down revolt after revolt.

After that civil war period ends, Alexander uses his new army consisting of conscripts from every corner of the Empire to return to India. This time, he knows his enemy and crushes the Indians. The spoils jump start his stagnating empire and fuel them for the invasion of China. China, however, is more advanced technologically but divided in civil war between the Eastern and Western Zhou. Alexander exploits the civil war to pit one side against the other and conquors China.

From there, he takes Korea, but it takes another 10 years before he can launch an attack on Japan due to a lack of naval resources.

In the west, however, Romans begin taking over parts of sicily. Alexander has to turn his attention West to fight the rising powers in the Mediterranean. The Japan campaign fails, but in the next 10 years, Alexander defeats Rome and Carthage.

When Alexander dies, his empire stretches from Spain to China.

If he somehow finds a strong successor, his empire continues to expand for at least one more generation until they literally run out of wealthy kingdoms to conquer causing an economic collapse and fracturing.
