Could be cancer, considering the unexplained weight loss. Losing weight without speed.
Maybe anemia due to iron deficiency, trying to amend his carnivorous habits (prior to the aforementioned Cheeseburger in Paradise).
Cannot rule out scurvy (vitamin C deficiency), since he was a Pirate 200 years too late, although the limes from the margaritas in Margaritaville make this less likely, not to mention the grapefruit consumed with the juicy fruit gum.

Dude is mid 70's and still tours a lot. Few entertainers have given to their fans as much as he has. Its amazing he can still do what he does so many days a year. My dad is 75 and in no where near that kind of shape
