Snoke is probably just a family friend, or family member, or instructor at Luke's academy. Somebody that had access to Ben Solo when he was young. His injuries were probably received in a fight, the night Kylo was wiping out the academy

That's how Luke grew up. He probably felt that level of isolation would keep her from being located on the gird, by a force user. Her longevity was far more important, than her living conditions

If you notice too, whenever Kylo Ren was in her mind trying to read the map to Luke he said he saw the water and the island. If that island is the same island Luke is on at the end of the movie then it's safe to assume Rey had been there before with Luke.
And it's no coincidence that the man who has the map to find Luke is living so close to Rey. He was obviously looking after her ala Obi wan

As seen, Jakku at one point was a battlefield for a fight between the rebellion and the Empire. It could also be just something she made based on something she came across, a la the rebel helmet she wears

I agree with some of this but the time lines of what we know make some of that hard to be true. Rey was left on Jakku when she was 5 or 6 and ben turned 6 years before the force awakens. That's when Luke goes into hiding

The only part of the vision she saw of the future was Kylo Ren in the snow on Star Killer Base though.

Rey is Han and Leia's daughter. She was born after they split unknown to Han.
