If you drink it toe pointing down you have to either tilt your head wayyy back and risk a spill to get the beer out of the toe, or turn the boot a complete 180 clockwise or counterclockwise to the sharper spout (where you would have started if you did toe up like in the movie).
Both ways would lead to too much beer coming out at one time or in one area, and there is the hassle of doing extra maneuvers that take more practice than the very short interruption of guiding a bubble over a quarter turn.

The times i have drank from a boot i did it sideways. No issues.
