Some of the celebrations are minor but it's clear they were documenting everything at that point to build their case. There are numerous serious infractions in there.
Getting a red card and then trying to play under a different name would have been the last straw at most facilities. That carries a serious ban. Lots of facilities require shin guards to play as a way of reducing liability. This is not unique. Typically not a serious situation but repeated infractions from an obviously known player are annoying.
Then there is the shite that makes the league unbearable for others. Like almost all businesses, the goal is to make money. Players that piss teams off and make a facility lose business are shitty and need to be dealt with.

Yes.
Maybe it's a typo and they meant 4. Or maybe the other team was really that bad.
Dumb rule though. Even if you want to have something like that, just make those goals not count. The coach will move you to defense after 3.
