Since we're including throwbacks:
Best -- old school Bears w/ the rounded numbers
Pat the Patriot red
Powder blue Chargers
Eddie LeBaron Cowboys
Worst -- too many to choose from, but the modern Patriots are certainly up there

I love how several of guys are putting the Cowboys uniforms in your top 5 worst. Only Cowboys haters do that. Cowboys uniforms have hardly changed over the years because annually they are in the top 5 in nationwide polls in a wide variety of publications and websites.
