Why are baserunners required to wear helmets but infielders/ pitcher do not?

Runners get hit in the head by first basemen going for the double play all the time.

Fielders are watching the ball, runners are not.

fielders are always looking at the ball. base runners are not. fielders have gloves and are allowed to use their hands to catch the ball or touch a live ball. base runners do not and cannot

Fielders have gloves to catch the ball that would otherwise frick their worlds up

They're also paying attention to the ball.
