When I get fatigued I tend to sway back in my backswing getting me off balance and swiping across the ball or hooking my downswing.
Started a slight pause at the top to make sure I'm balanced and ready to accelerate through the downswing.
Also used to roll my wrist and get too flat in my backswing when fatigued.

Misalignment. not a swing fault per se, but it causes some bad shots, nonetheless.
