Does the ps4 version support two people playing on the same console with one person as a guest or do you actually need two separate accounts?

Yes. you can both play but one will be a guest and their progress won't count towards their stats on their own account.

Well Antihero every time we try to play a two man game like Black Ops it asks for a PlayStation log in account. Like a PlayStation store account for player 2.

Just make a second one for your friends. That's what I did. It's free as long as the primary account has PS+.
