Happened at office Christmas party - bosses wife ended up being the one who picked it, both were approaching senior citizenship at the time, neither of them drink anymore, don't think they were amused either.

Witnessed this go down once.
Boss let the game play out, then made everyone announce which gift they brought, singling out the dildo-gifter.
Moral of the story, if you bring a dildo to a work White Elephant/Dirty Santa exchange, better bring 2 gifts and wrap them in different paper.
