I assume he saw it and thought Grealish was trying to buy a penalty, immediately blew the whistle, and then was stuck.
Maybe just slow down and see the immediate aftermath? It’s not like referees don’t do that with penalties and dives.
It looks like Grealish might have been a bit theatrical at the very end, but he was off balance. Just a weird call.
