Didn't witness the actual accident but I watched a Father carry his deceased six year old son into an emergency room after he was hit by a car while riding his bike. It was clear the child was gone and to see this man carry his whole world in his arms while crying,screaming, and moaning "my baby" repeatedly was more than I could take. I broke down myself and never got over witnessesing it. I still have nightmares about it till this very day.
That's my worst nightmare

Watched a friend of mine jump off the roof of an 11 story dorm building my freshman year at Tech. 1985
