I have health insurance on my dog through Healthy Paws. He just turned 1 and I have had him covered since I adopted him at eight weeks old. I did fairly extensive research and they are ranked at or near the top of most consumer reviews. I have a higher deductible as I only want coverage for unforeseen, high cost issues. I view my dog as part of my family.
I had my last dog for almost four years before we lost him. I adopted him as a puppy as well and pretty early on realized he had an intestinal disease. We managed it as best we could but lost him after about four years and a $5k surgery. We did not have him covered as I was not aware of pet insurance before his issues.
It's a personal decision. If you aren't willing to spend that kind of money (i.e. surgery) on your dog then you don't need insurance. But if you are, I think $40/month and peace of mind is worth it. Do your research though. Pet insurance is not a mature, competitive market like normal health insurance and I'm betting you can get taken advantage of if you go with the wrong provider.
Hope that helps.
