Based upon his story and how he recalls it, it was definitely a "pop."

The tenderness is mostly in his quadricep, but radiates to his knee and when watching him walk earlier today, he almost lost his balance due to instability.

May be time for him to pay a doctor or physical therapist a visit.

I'm willing to bet the doctor is going to tell him he has lost his MCL or its a high grade sprain.

You literally described what happens in an MCL injury.

Tell Bill that the pain in his quad is his body protecting the ligament injury. The quad is functioning in an atypical manner due to the instability of said ligament.
