They believe the mainstream media, but not their constituents when their constituents demand a repeal of Obamacare. Instead, like slowflo said, they are scared off by the MSM's absurdity with the "millions will die" bit.
And, the MSM has convince the people that healthcare = health insurance, and that Obama is about healthcare, when all it is, is a government sanctioned insurance plan paid for by the taxpayers.

Yes and no.
A lot of people benefit from Obamacare that have no idea that it is the reason for their benefits. 24% of rural Americans get coverage from the expansion of Medicaid - that is a HEAVILY pro-Republican section of the country.
The thing is, if city dwellers lose their coverage due to a Republican-passed mandate, then they will keep voting Democrat anyway. The few that voted Republican may switch.
Those that were able to get heart medication they otherwise could not afford under medicaid living in rural areas, OTOH, may flip. Enough of them do, and you could lose the Senate by 2018, the Presidency and probably the House by 2020.
The BEST way to get rid of it would be for a non-affiliated third party to appeal it to the Supreme Court and try to make the case that it is unconstitutional. IF they could pull that off, they could repeal the act without being blamed (Republicans).
