Certain errors are harder to avoid. In this case, twenty 150mg tablets of the same med were on the bottom of a large bottle with the correct 100mg tablets on top. The tablets were almost identical in size and the imprints were one number different. The patient takes 4 daily, which resulted in a 200mg overdose.
