That is a totally different situation.

If your co-worker is drinking on the job, you should take action. Use common sense to decide whether you report him or have a private conversation. Are people's livelihoods or personal safety in his hands?

If you are medical professionals, you are legally required report it.

Either confront him ASAP, or live with the guilt that you didn't.

Honestly, I've never had one on the clock, but I would consider one or two at a business lunch a little uncouth. That said, slinking out to the car for the hair of the dog = alcoholic.
