I got a bill last year saying I didn't pay withholding taxes for $850 (which I did) in 2014 with interest and penalties adding up to $1,350. I sent them a letter with my W-2 and my tax return from 2014.
I do my 2016 federal and state taxes and get part of my refund withheld for $1,350 by the IRS for LA Dept of Revenue. (Was in grad school and paid my max student loan interest so I got a large refund.)
I finally get a response from the LA Dept of Fyourlife and they basically said "My bad. To claim your refund back you must reply within 15 days with a refined return for 2014."
They also sent my father a notification that he overpayed $50 for 2015 saying he needs to refile to collect it. He declined.

Ha - this actually reminds me of another fun LDR story I went through. While in college I apparently just neglected to file a state return one year (I was working part-time). 2-3 years later the state sends me a notice saying I owed them some arbitrary amount in the range of $850 (seems to be their go-to catch all). I simply filed the state return and they ended up sending me a refund for $38.
