Well, you either make that person pay more or you take more from others in the form of higher premiums simply because you feel like it's fair.
Either works but claims are claims and must be paid. There's no getting around that.

I am guessing since the OP said "good lives" which at least insinuates lifestyle choices being the trigger.

Yes. There are large groups of people with some "x" factor who have a disadvantage in every marketplace since the beginning of time. It sucks. It ain't fair. But it's life. You frickers will destroy the planet just to make it fair. I mean if everyone is dead, then it's fair right?
