When the 15-22 was first released they no doubt had an issue with them. I had to send mine back twice to fix it, but its fixed now and has not skipped a beat since.
An Out of Battery issue is when the fired pin hits the primer before the bolt is all the way forward. The case ends up blowing out the side and its really loud. The first time it happened on my 15-22 it blew off the extractor. As well as scaring the shite out of me.
