Most of my clients have virus scans on incoming and outgoing email. In some cases, they are required to.

Edited to say most, not all. We don't test it for all of them.

Um. You don't know what you're talking about. Virus scan on the server OS and file system is a completely different thing than scanning incoming mail. Every single client scans mail with something like Ironport, proofpoint, etc and that sort of scanning is not done on the server. Those are separate appliances.
