It could be literally anyone still left alive by the end. Patsy, Little Carmine, Butch etc.
Plus he was about to be indicted, so it could have been someone from his own crew trying to keep him from squealing like Johnny Sack

Regardless what we think, the writer or creator did an interview a few years back and basically said the intent was that he died. The black screen was circling back to what tony envisioned for death.

Nothing. By definition, nothing happened after the story ended. There's not a secret truth you haven't been shown.
