There are theories that Patsy had made a deal with NY in the end and had given them Tony's location at the diner.
If you buy that there's a lot of foreshadowing in the final episode when Patsy and his wife visit the Sopranos at their house.

Havent' watched in forever, but I remember Tony and Carmela were not happy about Meadow dating Patsy's son and were uncomfortable about the whole dinner thing.
