This is my vote:
As for the final scene, I interpreted it that it was us (the audience) that was whacked. Remember earlier in that episode, someone said when you die, everything just goes dark. We were the ones that got killed. (That's my take, anyway.)

and this makes more sense to you than Tony? At the moment he should have seen someone enter the door? When the prior door openings all showed a person entering from Tony's perspective?
