It happened to my high school team. We ran out of the shotgun 70% of the time, including victory formation.
Standard pro formation with the half back and full back set about a yard ahead of the quarterback towards the line of scrimmage. We had a bad snap and it ricocheted off the fullback's kneepad. Minden recovered, but didn't score. Funny nonetheless.
