Agree on the suit. At a minimum the MD will have on a sportcoat, but probably a suit.
Also, if you need caffeine or a heavy breakfast to get moving, wake up early and consume that so you are at 100%. Just order light at the breakfast and opt towards something that doesn't require eating with your hands. Take a flip through the WSJ before the interview as well so you are up to speed on the latest business developments.
And of course, make sure to get enough sleep the night before.
