I've kind of always thought that "lawyer" connoted someone who has passed the bar exam and is allowed to practice law, while "attorney" connoted someone who is practicing law for a client.
Take the following similar sentences, for example.
This is Joe Smith. He is a lawyer.
This is Joe Smith. He is my attorney.
EDIT: I see I'm not the only one.

Lawyer is a general term for a person who gives legal device and aid and who conducts suits in court. An attorney or, more correctly, an attorney-at-law, is a member of the legal profession who represents a client in court when pleading or defending a case. In the US, attorney applies to any lawyer.
