chop up tomatoes, onions, peppers (jalapeno + whatever), cilantro, garlic. Then, add lime juice
pulverize some or all of it depending on your desired chunkiness
The portions of each are up to you. I like a lot of onion so I add more. My mom puts more cilantro in her bowl, bc she likes more. It's all about your taste.
ETA: you'll need salt. I use sea salt
