Last time I gave my kid medicine I had to do this. Ended up knocking out his loose tooth... there was cough syrup and blood all over the bathroom. It looked like a fricking crime scene.

I've perfected the one-arm headlock/arm-nullifier for my son. Luckily, I don't have to use it anymore. Once we got his tubes, he's rarely sick now and when he does take meds, he's good about just letting it happen.
