They never explain anything. Similar to Clara jumping into the Doctor's time stream then the next time we see her, they pretend nothing happened.

I didn't think Missy died in her last appearance in the cemetery, I thought she teleported away just before the Doctor could kill her. Could be wrong.
Everything else, well..it's timey wimey stuff. I find the show much more enjoyable if I don't try to make too much sense of it.
