Can you do an exchange on something bigger than a 20 LB tank? I don't think I've ever seen that.

Yes. Most gas supply places will swap your empty for their full provided its in test.
There should be dates stamped on the top indicating the last time it was hydro tested. I believe propane tanks have to be tested every five years. Amerigas, Airgas, Praxair should all swap a PR 40. I'd call ahead though to see if they've got one on the yard.
If your tank isn't in test, they'll still swap it but they'll charge you a hydro fee
And yes, it's always easier to swap your empty for their full instead of getting one filled
